Provider Networks and Direct Billing

Countries Where Cigna Won’t Pay

If Your International Claim Is Denied

Read the explanation of benefits carefully. It lists the specific reason for denial, and that reason drives your response. International claims stumble most often on documentation gaps, missing preauthorization, or disputes over medical necessity.

You generally have 180 days from the denial date to file a formal appeal, though your specific plan may allow longer.9Cigna Healthcare. Medicare itself almost never pays for care outside the United States.
