LensCrafters accepts most major vision insurers as in-network providers, including EyeMed, Aetna, Humana, UnitedHealthcare Vision, Superior Vision, HMSA, Davis Vision, Cigna, MetLife, BCBS FEP Vision, Spectera Vision, Vision Benefits of America, and Wellpoint.1LensCrafters. Insurance Plans The one carrier that surprises people: VSP, the largest vision insurer in the country, is not in-network. Whether your plan is in-network or out-of-network is the single detail that decides what you pay, because the difference on the same pair of glasses can run into the hundreds.
The In-Network List
For any plan on the list above, LensCrafters applies your benefits directly at checkout, both online and in stores. No claim form, no waiting for reimbursement. You pay your copay and your share of anything above your allowance, and you walk out.1LensCrafters. Insurance Plans
The list above reflects what LensCrafters publishes on its website. Individual stores may accept additional carriers through local agreements, so if your plan is not listed, call the nearest location before assuming you cannot use it there.
VSP Members Pay Out-of-Network Rates
VSP appears on the LensCrafters website only as an out-of-network carrier.1LensCrafters. Insurance Plans If you have VSP, you pay full retail at LensCrafters and then submit a claim to VSP for partial reimbursement. That reimbursement is typically smaller than what you would save by going to a VSP in-network provider like Costco Optical or many independent optometrists.
LensCrafters does give VSP members a 20% discount on items the plan does not cover, but the discount does not apply to professional services or contact lenses.1LensCrafters. Insurance Plans VSP allows up to 12 months from the date of service to submit a reimbursement claim.2VSP Vision Care. File a Claim for Reimbursement
Why EyeMed Stretches the Furthest
EyeMed consistently offers the deepest discounts at LensCrafters, and the reason is corporate: both companies are owned by EssilorLuxottica. LensCrafters is the group’s flagship retail chain and EyeMed is its insurance arm. That common ownership shows up in higher frame allowances and lower copays on lens upgrades than other in-network insurers typically deliver here.
If your employer lets you choose between EyeMed and another vision plan and you know you will shop at LensCrafters, EyeMed is almost always the better fit. The reverse holds too: if you already have EyeMed, LensCrafters is where the benefit stretches the furthest.
Medicaid and Medicare
Medicaid covers vision care for enrolled children, including eye exams, eyeglasses, and medically necessary treatments, as part of the federal Early and Periodic Screening, Diagnostic, and Treatment benefit.3Medicaid.gov. Vision and Hearing Screening Services for Children and Adolescents Adult coverage varies by state. A National Institutes of Health study found that about 12% of Medicaid enrollees lived in states with no coverage for routine adult eye exams, and 27% lived in states that did not cover eyeglasses at all.4National Eye Institute. Medicaid Vision Coverage for Adults Varies Widely by State Whether LensCrafters accepts your state’s Medicaid plan depends on local provider agreements, so check with both your state Medicaid office and your nearest LensCrafters store.
Original Medicare (Parts A and B) does not cover routine eye exams.5Medicare.gov. Eye Exams (Routine) It also does not cover eyeglasses or contact lenses except after certain surgeries such as cataract removal.6Medicare.gov. Eyeglasses and Contact Lenses Medicare Advantage (Part C) plans often bundle vision benefits, and LensCrafters accepts Medicare Advantage Flex Cards from carriers like Aetna and Humana toward exams and eyewear, in stores and online.1LensCrafters. Insurance Plans Coverage varies by plan, so confirm with your Advantage carrier before scheduling.
Paying With an FSA or HSA
Even with no vision insurance, a Flexible Spending Account or Health Savings Account can cover most of what you would buy here. LensCrafters accepts FSA and HSA cards online and in stores for eye exams, prescription eyeglasses, prescription sunglasses, and contact lenses.7LensCrafters. FSA and Prescription Eyewear You swipe the card at checkout like a debit card.
The IRS treats eye exams as eligible medical expenses for HSA, FSA, and HRA reimbursement because they diagnose disease or illness.8Internal Revenue Service. Frequently Asked Questions About Medical Expenses Related to Nutrition, Wellness and General Health Prescription eyeglasses and prescription sunglasses also qualify under Section 213(d) of the tax code. Non-prescription items like plano sunglasses or blue-light glasses without a prescription do not qualify.
For 2026, the HSA contribution limit is $4,400 for self-only coverage and $8,750 for family coverage, and the health FSA salary reduction limit is $3,400.9Internal Revenue Service. Revenue Procedure 2025-19 If you have both vision insurance and one of these accounts, the tax-advantaged funds can cover copays, any amount above your frame allowance, and lens upgrades your plan does not fully cover.
In-Network vs. Out-of-Network at the Register
When LensCrafters is in-network for your plan, the store has a pre-negotiated agreement with the insurer. Benefits apply automatically at checkout: the copay is lower, the frame allowance is higher, and lens add-ons like anti-reflective coating or progressive lenses cost less. You pay your share and leave.
Out-of-network flips the process. You pay full retail at the register, file a claim with your insurer afterward, and wait for a partial reimbursement that is almost always smaller than the in-network discount would have been. A plan that covers $200 toward frames in-network might reimburse only $100 out-of-network, and some plans apply a separate, higher deductible to out-of-network services. The math rarely favors you unless the specific frames you want are only sold at LensCrafters.
Check Your Benefits Before the Appointment
LensCrafters has an online insurance verification tool. You select your carrier and enter your name, date of birth, and zip code, and the system pulls your available benefits in real time.1LensCrafters. Insurance Plans The information is only available during your browser session and is erased when you close the window, so screenshot or write down anything you want to keep.
You can also call your insurer and ask two questions directly: is LensCrafters in-network for your plan, and how much of your benefit remains for the current plan year. Some plans reset on January 1, others on your enrollment anniversary. Unused allowances near the end of the year disappear if you do not use them.
If You Have to File an Out-of-Network Claim
Ask LensCrafters for an itemized receipt that lists the exam fee, frame cost, lens cost, and each lens add-on separately. Insurers need that breakdown to process the claim. Claim forms live on the insurer’s website, and you will need your policy number, the provider’s name and address, the date of service, and the itemized expenses. Some carriers also require diagnosis or procedure codes, which the optometrist at LensCrafters can provide on request.
Filing deadlines vary. VSP allows 12 months from the date of service,2VSP Vision Care. File a Claim for Reimbursement and EyeMed also allows a year for out-of-network claims. Other carriers may give you less time, so check your plan documents or call member services. Miss the deadline and the reimbursement is gone.
