Humana is one of the largest health insurance companies in the United States, best known for its Medicare Advantage plans, which cover roughly 7 million people. To answer what Humana insurance is in plain terms: it’s a private insurer that sells individual and family plans on the Affordable Care Act marketplace, employer group coverage, Medicare Advantage and Part D prescription drug plans, and Medicaid managed care in select states. What any given Humana plan covers depends on the product and tier, but every plan must meet a federal floor that includes preventive care, emergency services, and protection against surprise medical bills.
What Humana Sells
Humana’s product lineup splits along who you are and how you get coverage. If you’re under 65 and buying on your own, you’d look at an ACA marketplace plan. If you get insurance through work, your employer may offer a Humana group plan. If you’re 65 or older or otherwise Medicare-eligible, Humana sells Medicare Advantage plans (its largest business), standalone Part D drug plans, and Dual Eligible Special Needs Plans for people who qualify for both Medicare and Medicaid. The Plan Structures: HMO, PPO, and PFFS
Whichever category you fall into, the specific plan you pick has a structure that decides which doctors you can see and what you pay out of network. Humana’s three main Medicare Advantage structures are HMO, PPO, and Private Fee-for-Service.
- HMO plans require you to pick a primary care physician who coordinates care and refers you to specialists. Out-of-network care generally isn’t covered except in emergencies. Premiums tend to be lower.
- PPO plans don’t require referrals and do cover out-of-network providers, though you’ll pay more for going outside the network, often including a separate out-of-network deductible.

The practical tradeoff is cost against flexibility. HMOs cost less if you’re comfortable inside a network. What’s Covered

Preventive care sits in its own category. Medicare Advantage plans include all of Parts A and B, and many add routine dental exams and cleanings, vision coverage with frames and lenses, hearing exams, and hearing aids. Extras on Some Medicare Advantage Plans
Several Humana Medicare Advantage plans include SilverSneakers, a fitness benefit for adults 65 and older that covers participating gyms, online classes, and on-demand workouts. Some plans include it at no additional cost; others charge a small monthly fee.6Humana. For members who qualify for both Medicare and Medicaid, Humana’s Dual Eligible Special Needs Plans coordinate benefits from both programs. Many D-SNPs offer $0 copays on covered Part D drugs (including brand names) with no deductible, plus routine dental, vision, and hearing.8Humana. How Prescription Drugs Are Covered
Humana uses a five-tier formulary. Tier 1 is preferred generics at the lowest cost. Tier 2 is other generics. Tier 3 is preferred brand-name drugs. Tier 4 is non-preferred drugs at higher cost-sharing. Tier 5 is specialty medications with the highest out-of-pocket costs.9Humana Inc. Medicare enrollees have a hard annual out-of-pocket cap on Part D drugs. In 2025 the cap was $2,000; for 2026 it rises to $2,100 after an annual adjustment. Once you hit that amount, covered Part D drugs cost nothing more for the rest of the year. What You’ll Pay
Humana plans use the standard cost-sharing layers: a monthly premium, an annual deductible before most non-preventive coverage begins, copayments for specific services, and coinsurance that splits the bill on a percentage basis. Actual amounts vary widely by metal level (for marketplace plans) or by specific Medicare Advantage plan.
ACA marketplace plans in 2026 cannot exceed an out-of-pocket maximum of $10,600 for an individual or $21,200 for a family. Once you spend that much on covered in-network care in a plan year, Humana pays 100% of remaining covered costs.12HealthCare.gov. For Medicare enrollees, the standard Part B premium in 2026 is $202.90 per month with a $283 annual deductible. Subsidies for Marketplace Plans
If you buy a Humana plan through HealthCare.gov, you may qualify for a premium tax credit that lowers the monthly cost. For 2026, eligibility returns to the standard range: household income between 100% and 400% of the federal poverty level. The temporary expansion that removed the 400% cap between 2021 and 2025 has expired. Some people who received larger subsidies in recent years will face higher premiums in 2026 unless they change plans.14Internal Revenue Service. When You Can Enroll
Enrollment windows depend on the product. Miss one and you generally wait until the next annual period unless a qualifying event opens a special window.
ACA Marketplace
Open Enrollment runs from November 1 through January 15. Enroll or switch by December 15 and coverage starts January 1. The Medicare Annual Election Period runs from October 15 through December 7. Changes made then, whether switching from Original Medicare to a Humana Medicare Advantage plan or changing Part D coverage, take effect January 1. Protections You Have as a Member
Federal law bars Humana from retroactively canceling your coverage once you’re enrolled unless you committed fraud or intentionally misrepresented material facts. Cancellation requires prior written notice.
The No Surprises Act protects you when you receive emergency care or certain out-of-network services at in-network facilities. In those situations, you can’t be billed beyond your normal in-network cost-sharing, emergency room care is treated as in-network regardless of which hospital you end up at, and any cost-sharing you pay counts toward your in-network deductible and out-of-pocket maximum.18Centers for Medicare & Medicaid Services. If Humana denies a claim, you get an Explanation of Benefits showing the reason. Simple billing errors, like an incorrect procedure code, can often be fixed by asking your provider to resubmit.
