Oscar Health is a technology-focused health insurance company that sells Affordable Care Act plans to individuals, families, and small businesses in roughly 20 states. Founded in 2012 and headquartered in New York, Oscar built its business around an app-driven experience, virtual care at no extra cost, and dedicated care teams that help members navigate the healthcare system.1Oscar Health Inc. What Makes Oscar Different

Oscar started selling individual plans in 2014 after launching in 2012, initially only in New York before expanding to other states.2Oscar Health Insurance. About Us From the beginning it leaned on features that traditional insurers didn’t emphasize: a member app that tracks claims and deductible spending in real time, 24/7 virtual care, and a concierge-style Care Team assigned to each member. The Care Team handles appointment booking, billing questions, and referrals, so members don’t have to call a general customer service line and start the conversation over each time.

The clearest example of the technology bet is Virtual Primary Care, which is included on most Oscar plans at no additional cost. A dedicated virtual provider can handle routine visits, order lab work and imaging, manage prescriptions, and refer you to specialists. When labs are ordered through the virtual provider, Oscar sends a phlebotomist to your home, and members receive a vitals monitoring kit so the provider can track key health metrics remotely. On many Oscar plans, prescriptions the virtual provider writes for generic drugs, lab work, basic imaging like mammograms and X-rays, and even initial specialist visits carry a $0 copay.3Oscar Insurance. Where You Can Buy an Oscar Plan

Oscar does not sell plans in every state. For the 2026 plan year, individual and family plans are available in roughly 21 states: Arizona, Alabama, Florida, Georgia, Illinois, Iowa, Kansas, Michigan, Mississippi, Missouri, Nebraska, New Jersey, New York, North Carolina, Ohio, Oklahoma, Pennsylvania, Tennessee, Texas, and Virginia.4Oscar Health. 2026 Transparency in Coverage Documents Even within those states, Oscar typically covers specific counties or metro areas rather than the entire state, and the footprint changes from year to year. Small group plans have a different footprint and may be available in additional markets through partnerships.

Before you start comparing plans, confirm your county is in an Oscar service area. Oscar’s website and HealthCare.gov both show which plans are available at your address.

The Plans Oscar Sells

Like all ACA insurers, Oscar sells plans in four metal tiers. Each tier reflects how costs are split between you and the insurer on average across a population:

  • Bronze plans cover roughly 60% of average costs. Lower premiums, higher deductibles and copays.
  • Silver plans cover about 70%. A middle ground on premiums and out-of-pocket costs, and the only tier eligible for cost-sharing reductions.
  • Gold plans cover about 80%. Higher premiums, lower deductibles and copays.
  • Platinum plans cover about 90%. The highest premiums, the lowest costs at the point of care.

Every Oscar plan must cover the ten categories of essential health benefits required by federal law: outpatient care, emergency services, hospitalization, maternity and newborn care, mental health and substance use treatment, prescription drugs, rehabilitative services, lab work, preventive care, and pediatric services including dental and vision for children.5eCFR. 45 CFR Part 156 Subpart B – Essential Health Benefits Package Preventive services like annual physicals, certain screenings, and vaccinations are covered at no cost, even before you meet your deductible.

Every plan also caps annual out-of-pocket spending. For 2026, the federal maximum is $10,600 for individual coverage and $21,200 for a family plan.6HealthCare.gov. Networks and How You Get Care

Oscar primarily uses EPO and HMO network models, depending on the state. HMO plans are offered in states like New York, Texas, Georgia, and Arizona; certain Florida markets get EPO plans instead.7Oscar Health Insurance. HMO plans may require a referral from your primary care provider before you see a specialist. EPO plans typically do not.9Oscar. If you end up in an out-of-network emergency room, or an out-of-network provider treats you at an in-network facility without your knowledge, the federal No Surprises Act limits what you can be charged. Your cost-sharing in those situations can’t exceed what you would have paid in-network, and providers cannot balance-bill you for the difference.10Centers for Medicare & Medicaid Services. Prescription Coverage

Oscar uses a tiered formulary. Dollar amounts depend on your plan, but the structure is consistent:

  • Tier 0: select preventive medications at $0.
  • Tier 1A and 1B: generic drugs at the lowest cost. On some plans, Tier 1A generics run as little as $3.
  • Tier 2: preferred brand-name drugs or higher-cost generics at a moderate cost.
  • Tier 3: non-preferred brand-name drugs at a higher cost.
  • Tier 4 and 5: specialty medications at the highest cost-sharing level.

The formulary is published online, so you can search for your medications before enrolling to see which tier they land on and whether prior authorization or step therapy applies. If your doctor prescribes a drug that isn’t on the formulary or sits on a higher tier, you can request an exception through Oscar’s appeals process.

What Oscar Plans Cost

Because Oscar sells on the ACA marketplace, its plans are eligible for the same federal subsidies as any other marketplace insurer. Two kinds of help can dramatically lower what you actually pay.

The Advance Premium Tax Credit reduces your monthly premium on a sliding scale based on household income relative to the federal poverty level. Under the ACA’s permanent rules, you qualify if your household income falls between 100% and 400% of the federal poverty level. From 2021 through 2025, temporary legislation removed the 400% cap, opening credits to higher-income households.13Centers for Medicare & Medicaid Services. As of early 2026, Congress was considering an extension. If you earn above 400% of the poverty level, check HealthCare.gov or Oscar’s enrollment tool for the latest on whether enhanced credits apply to your plan year.

Cost-sharing reductions are the second form of help. They lower your deductibles, copays, and out-of-pocket maximums, but only on Silver-tier plans and only if your income is between 100% and 250% of the federal poverty level. The savings are substantial: for someone earning below 150% of the poverty level, the annual out-of-pocket maximum drops from $10,600 to as low as $3,350 for individual coverage. At 200% to 250% of poverty, it drops to $8,100.14Congressional Research Service. Signing Up for an Oscar Plan

Oscar follows the same enrollment rules as every other marketplace insurer. Open Enrollment runs from November 1 through January 15.15HealthCare.gov. When Can You Get Health Insurance? Enroll by mid-December and coverage typically starts January 1. Enroll in January and coverage starts February 1.

Outside that window, you need a qualifying life event to trigger a Special Enrollment Period. Small businesses can generally enroll employees year-round, provided they meet participation requirements set by the insurer and the state.

Paying Premiums and Staying Covered

Oscar accepts payment through autopay in the app or on the website, online payment with a checking account or debit card, phone payment with a checking or savings account, or a mailed check or money order. Oscar does not accept credit cards for premium payments.17Oscar Health Insurance. How Do I Pay My Bill? Autopay is the simplest way to avoid a missed payment, because the consequences get serious quickly.

If you receive premium tax credits and miss a payment, federal rules give you a three-month grace period, provided you’ve paid at least one full month’s premium during the benefit year.18HealthCare.gov. During the second and third months, Oscar can hold claims and notify your providers that payment may be denied.19eCFR. 45 CFR 156.270 – Termination of Coverage or Enrollment for Qualified Individuals If you don’t pay up before the grace period ends, coverage is terminated retroactively to the end of month one, and you’re personally on the hook for anything you received in months two and three.

If you don’t receive premium tax credits, your grace period may be shorter, depending on state insurance rules. And losing coverage due to nonpayment generally does not qualify you for a Special Enrollment Period, so you could be uninsured until the next Open Enrollment.18HealthCare.gov. If a Claim Gets Denied

You usually don’t file claims yourself. Your doctor or hospital submits them to Oscar, which applies your deductible, copay, or coinsurance and pays the provider at the negotiated rate. You can track pending and processed claims through the Oscar app.

When Oscar denies a claim, you get a written explanation. Common reasons are that a service was found not medically necessary, fell under a plan exclusion, or was billed with incorrect codes. You have 180 days from the denial to file an internal appeal, which Oscar reviews with different personnel than those who made the original decision.

If the internal appeal is denied, you can request an external review by an independent third party with no ties to Oscar. The federal deadline for requesting external review is four months from the date you receive the final internal denial.20HealthCare.gov. For urgent situations involving ongoing treatment, you can sometimes request an expedited external review that bypasses the internal process. Attaching supporting documentation like medical records or a letter from your treating physician strengthens any appeal.
